Our GMH Rehabilitation Team works together to ensure continuity of care and learning. Goals are designed with the patients to help them achieve the highest level of independent functioning in all areas.
Audiologist
The Audiologist specializes in prevention, identification, assessment and rehabilitation of hearing disorders.
The Audiologist prescribes and assists in obtaining hearing aids. Hearing-impaired patients and their families are instructed in the use of the hearing aid. If further follow-up care is needed, the audiologist will make recommendations to meet the patient's needs.
Clinical Dietitian
The Dietitian will provide an initial nutritional screening on each patient admitted to the Rehab Unit. At that time, nutritional needs such as weight loss, weight gain or dietary education will be addressed. If additional services are needed, the physician may order a dietary consult. For special food concerns, please feel free to ask the dietitian.
Occupation Therapist
The Occupation Therapist work with the patient/family/caregiver to improve function and self-care abilities. Therapy may focus on problems in the areas of feeding, dressing, bathing, toileting, upper body range of motion and strength and mobility in daily activities or homemaking. The therapist may visit the home with the patient to recommend modifications for safety. The therapist is also able to provide a driver's screening when potential problems appear that may affect driving skills.
Physiatrist
Medical care on the Rehabilitation Unit will be directed by a Physiatrist. A physiatrist - or "fizz-ee-at-trist" - is a doctor who specializes in physical medicine and rehabilitation.
The physiatrist will work with your family doctor and consultants to continue your medical care, as well as evaluate and treat problems or complications. The physiatrist will provide medical direction to the rehabilitation services and therapies you receive.
The physiatrist will be available for follow-up care. Prior to discharge, plans are made for continuing care with your family doctor. A discharge summary and medical records will be provided.
Physical Therapist
The Physical Therapist evaluates and develops treatment programs for patients with impairments or changes in physical function resulting from injury, disease or other causes. The therapist offers selective services that are necessary to maximize activity levels for mobility, work and recreation. The patient learns from a team approach using various techniques to improve the patient's ability to move in bed, propel a wheelchair, transfer, stand and walk. The therapist provides patient, family and caregiver instruction and education to prepare for transition to home, work and the community.
Rehabilitation Psychologist
The Rehabilitation Psychologist is available to provide emotional support and counseling to patients and families. Many times, rehabilitation involves adjusting to change and dealing with losses, as well as recognizing one's strengths and abilities.
Patients may also benefit from learning techniques to more effectively manage pain and discomfort. Both individual and group sessions are a part of the program. The psychologist may also assess the thinking and memory skills of each patient. This information is conveyed to the patient and the team to increase awareness of how each person can learn most effectively.
Rehabilitation Inpatient Services
Welcome to the GMH Rehabilitation Unit., This unit is contained within the hospital, but is distinctly different from other nursing units. The physician, along with the "
Rehabilitation Team", has chosen this unit for the patient to increase his or her level of independent living.
The Physical Medicine and Rehabilitation team at GMH works with patients who have had a variety of illnesses or injuries resulting in physical and/or cognitive difficulties. The team of professionals recognizes the uniqueness of each individual and views each person in his or her own environment to provide specific, personalized programs in patient care and education. We provide a compassionate and holistic plan of treatment that includes the family and/or caregiver and continues after discharge.
The rehabilitation team is dedicated to helping patients and their families achieve an optimal, functional status and the highest quality of life possible. Our small, friendly, family-oriented unit provides an atmosphere that responds to the needs of the surrounding community. The team operates within the accreditation requirements of JCAHO, CARF and other regulatory agencies, as well as seeking input to evaluate/improve program effectiveness.
Rehab Nursing Staff
The Rehab Nursing Staff works closely with all therapy areas to achieve consistent patient care. In addition to traditional nursing care, the patient, family and caregivers receive individualized instruction.
Topics may include self-medication programs, diabetic teaching, skin care and bowel and bladder training programs. The nurses will coordinate your nursing care while your on the GMH Rehab Unit and will call you after your discharge to follow up with you.
Rehab Social Worker
An injury or illness that requires rehabilitation can have a great impact on patients and their families. The social worker can help with adjustment issues, financial concerns, emotional needs and discharge planning. The social worker serves as a case manager who coordinates the provision of care.
As a liaison and advocate, the social worker will communicate the patient's progress to family members, as well as family concerns and goals to the treatment team. Family/caregiver participation in therapies and therapeutic day/overnight passes may also be arranged by the social worker.
Speech Pathologist
The Speech Pathologist evaluates and treats communication, cognitive and swallowing skills. Following the initial assessment, an individualized therapy program is designed to focus on recovery or maintenance of speech language and cognitive functions.
Cognitive functions may include memory, problem solving, organization, as well as higher-level thinking and reasoning skills. If chewing or swallowing problems are identified, adjustments to the patient's diet may be made, as well as instructing the patient on safe swallowing techniques.
In addition, patients attend a weekly communication group to maximize their communication and social interaction skills in natural settings.
Therapeutic Recreation Specialist

The Therapeutic Recreation Specialist will evaluate each patient's needs to increase independence and awareness of leisure skills. Recreational skills will be utilized to increase communication, cognitive, physical and social skills.
Therapeutic community outings, garden therapy and canine-assisted therapy may be used with assistance from other members of the treatment team to increase the patient's independence. special events and holiday functions will also be developed in conjunction with individual needs, goals and objectives.
*On the day of admission, the family is needed during the admitting and evaluation process. Visiting hours are not enforced this day.
*Daytime hours are devoted to therapy. Therapy is not to be interrupted for visitations.
