1. Induction Phase

A. Orientation - Discussion of behavioral aspects, medical benefits and risks, and financial aspects of program. Those attending receive detailed information about the program

B. Medical Assessment - Laboratory testing, Health History, EKG and Physical Exam. Clinical information is gathered and patient is approved for weight loss phase. Level of medical supervision is determined.

2. Weight Loss Phase

A. Program Track

1. Medical Track (levels determined by initial body weight and medical status)

Medically-supervised
RX: The Very Low Calorie Diet or Low Calorie Diet - The minimum of 520-800 calories per day using HMR meal replacements.

Intermediate
RX: Low Calorie Diet - minimum of 800 calories per day using HMR meal replacements.

Moderate
RX: Low Calorie Diet - minimum of 800 calories per day using HMR meal replacements.

- Meal Replacements consist of: Chocolate & Vanilla supplements, Chicken Soup and entrees -

2. Non Medical Track

Healthy Solutions (recommended for those without medical problems who want to lose 40 lbs or less)

Reduced Calorie Diet - Consists of 3 supplements, 5 servings fruits/vegetables, 1-3 entrees - total of 1200-1600 calories per day.

B. Introductory Classes (two weeks)
Patients are introduced to program structure and policies. Plans and procedures for weight loss success are established. No Meal Replacement sales during this time.

C. Weekly Educational / Behavioral Group (24 weeks)
attendance, record keeping and weekly phone calls are mandatory commitments.

Formal weight loss and product sales begins; patients on the medical track must purchase 2 boxes of supplement per week. Patients are taught the basics of weight control and are helped to develop self- management skills. The focus will be on practicing behaviors that facilitate weight management. Topics include goal setting, record keeping, environmental control, strategies for success, communication skills, plan making, problem-solving relapse management, physical activity and the HMR calorie system. Individual support and coaching given throughout the program.

3. Transition Phase (approx. 3-6 weeks)
Transition from very low calorie diet or low calorie diet to regular food. Refeeding plans are made with your behavioral educator. Patient stays under medical supervision until refeeding period is concluded.

4. Maintenance Phase (12-36 months)
Weekly classes and support- the most important phase of the program where patients develop the skills necessary to establish a healthy lifestyle and maintain a lower body weight. All topics discussed in the weight loss phase are further explored. Low-fat eating, cooking, grocery shopping, restaurant strategies are discussed. Individual support and coaching continue. The Triple Imperative ( 14 meal replacements, 35 servings of fruits and vegetables, and 2000 physical activity calories per weeks) is mandatory.
